At Valley Medical Primary Care we strive to deliver excellent patient care. Your health is our number one priority. We are pleased that you have chosen us for your primary medical care needs. We would like to take a few minutes to introduce you to our practice and share with you about some of our office policies. We have three physicians and three nurse practitioners who are helped by office staff including medical assistants and receptionists etc who all work hard to accommodate your medical needs.
The following are some office policies and procedures and important contact information:
Office Hours ::
Our office hours are Monday through Thursday 8:30 a.m. to 5:00 p.m. and Friday 8:30 a.m. to 3:00 p.m. We are closed on all major holidays.

Scheduling Appointments ::
To schedule an appointment, call (937) 208-8282 and select option #1. You will be then directed to one of our receptionists who will be happy to schedule an appointment for you.
At each appointment, please bring your insurance card(s) for verification and a current list of your medications.

In case of an Emergency call 911.
If you need medical attention in after hours or on the weekend, you can contact the physician through Medical Society at (937) 463-1700. Please do not contact the physician for prescription refills in after hours. Refills have to be requested through the office in regular working hours by calling the prescription line.

Prescription Refills ::
Your physician will write you prescriptions with a specific number of refills. We recommend that they day you receive your last refill; you call the office to schedule your next appointment. It is important that your physician sees you periodically so he/she can evaluate how you are responding to the medication and make adjustments as needed.

- Please Allow two business days for your prescriptions to be called into your pharmacy
- Do not leave prescription requests on the nurses’ lines. We will process prescription requests only from the prescription line.
- Please do not leave prescription requests with the receptionists.
›› Requests for Antibiotics
It is the policy of the physicians at Valley Medical Primary Care not to prescribe antibiotics over the telephone. It is very important for you to be seen when you think you have an infection. Please understand that when you call requesting an antibiotic, the receptionist will request that you make an appointment.
›› Requests for Scheduled Drugs
If you need any scheduled prescriptions (i.e. narcotics or anxiety medications) you will need to see the doctor before the prescriptions can be refilled. This is regulated and monitored by the State of Ohio
Payment for Service ::
Co-pays and any previous balances are due at the time of service. We will be happy to provide you with a copy of your statement.
